ENHANCEMENT: rewrote the user defined forms cms JS into jquery. ENHANCEMENT: rewrote templates and removed dulicate code. APICHANGE: created parent class for EditableFormField, EditableOption and EditableMultiOption to prevent code duplication. MINOR: added icon for literal field. MINOR: added quick unit test.
