Removed version numbers from OS req docs

Too icky to keep updated as version numbers change. It's really just a matter of what PHP version supports which OS, from a SilverStripe side we don't have any constraints there. Well, technically Windows 3.11 won't work because the filesystem will fall over, but otherwise LAMP/WAMP is pretty forgiving.

JIRA isn't fully under the OSS team's control,
and played up in the past (Dan couldn't move issues).
Since Github has project boards now, and we're paying
for private repos on github.com/silverstripe-security already anyway,
there's no reason to introduce another tool (JIRA) into our workflows.
No need to move existing issues, the JIRA board hasn't been used in a while.
Which leads to unclear ownership and status of security issues,
and is exactly the reason for this change ;)

The previous exmaple for adding a Mamcached backend was not working properly. Using this example results in Zend_Cache_Backend_Memcached using the default (fallback) settings (which are exactly the same as in the exmaple though).
+ Added an extra example for Memcached instances which are listening on a Unix socket.

- Use "Removed/Changed/Added/Moved" prefixes to make content more scannable
- Removed "change" vs "remove" distinction in subheadings (hard to separate, and not meaningful in practice)
- Linearised messages, less nesting - easier to parse (don't need to remember context from a few lines above)
- Simplified language (e.g. "use" instead of "please use")
- Simplified headline wording (remove repetitive "upgrade code that uses X")
- More anchor links across sections
- More use of diffs to clearly communicate before/after states
- Consistently apply backtick code formatting
- Consistently use "()" to denote methods (easier to read)
- Removed duplicate SQLQuery instructions (and simplified the augmentSQL example)
- Add new composer.json intro

Remove rewrite-less routing through index.php

It's currently broken (doesn't rewrite subsequent links),
and is of questionable use. It was introduced during a time
when PHP didn't have a built-in webserver (I think).
Virtually ever webserver will have rewriting capabilities these days (even IIS!),
and if you struggle with the setup as a new user, you can just fall back to PHP's built-in webserver.
This doesn't affect installation capabilities, since these are triggered via install.php.

Remove TinyMCE spellchecker instructions
...
They're broken since you can't access PHP files in the vendor dir any more,
and they're not easily fixed by creating a new rpc.php file in the webroot.
There's something around TinyMCE missing a base_uri.
Regardless, browsers have great built-in spellcheck these days,
so server-side spellchecking is more of an edge case that we don't have to cater for in core.

Changed the `strictFormMethodCheck` protected property from false to true to step out on the front foot with this security setting. In the documentation under the title [Cross-Site Request Forgery](https://github.com/silverstripe/silverstripe-framework/blob/master/docs/en/02_Developer_Guides/09_Security/04_Secure_Coding.md#cross-site-request-forgery-csrf ) it states, "it is also recommended to limit form submissions to the intended HTTP verb (mostly GET or POST) through [api:Form::setStrictFormMethodCheck()]." The same advice is noted in [Form Security](c2292a4cc1/docs/en/02_Developer_Guides/03_Forms/04_Form_Security.md (strict-form-submission)14c59be8

API: Add HTTPOutputHandler::setCLIFormatter
Fixes https://github.com/silverstripe/silverstripe-framework/issues/6835
This provides detailed errors (but not warnings or notices) in CLI calls
on live environments.
It does this by adding a 2nd argument to our output handler,
CliFormatter. This formatter will be used when Director::is_cli() is
true.

Fixed timezone and normalised ISO handling

A few observations:
- ISO says “T” is optional (https://en.wikipedia.org/wiki/ISO_8601#cite_note-21 ),
- WHATWG says in the HTML5 spec that it’s optional (https://html.spec.whatwg.org/multipage/infrastructure.html#local-dates-and-times )
- W3C says it’s reqiured in 1997 (https://www.w3.org/TR/NOTE-datetime ), but then later says it’s optional in its HTML5 spec (https://www.w3.org/TR/html5/infrastructure.html#floating-dates-and-times ).
- Chrome doesn’t parse values with whitespace separators (requires "T")
- DataObject DBDatetime values and database columns use whitespace separators (and will have many devs relying on this format)
- MySQL only supports whitespace separators (https://dev.mysql.com/doc/refman/5.7/en/datetime.html )
- SQLite can parse both ways (https://sqlite.org/lang_datefunc.html )
So the goal here is to retain ORM/database compatibility with 3.x (whitespace separator),
while exposing "T" separators to the browser in HTML5 mode.
Regarding timezones, this fixes a regression where setValue() would not actually
apply the timezone (last $value assignment is ineffective now that sub fields are removed).
