feat: implement QuestionBank CRUD with pagination and template query
- Add pagination support to findAll (page, limit query params) - Add findByTemplateId method to service - Add GET /by-template/:templateId endpoint to controller - Service already includes CRUD for QuestionBank and QuestionBankItem
